Der Gastrointestinale Mikrobiom Test Plus (GI-MAP) ist ein innovatives klinisches Instrument, das die DNA der gastrointestinalen Mikrobiota aus einer einzigen Stuhlprobe mit modernster quantitativer Polymerase-Kettenreaktion (qPCR)-Technologie misst.

Das GI-MAP wurde entwickelt, um Mikroben zu erkennen, die das normale mikrobielle Gleichgewicht stören oder zur Krankheit beitragen können, sowie Indikatoren für Verdauung, Absorption, Entzündung und Immunfunktion. Mit einem kulturbasierten Test ist es nicht möglich, strikte Anaerobier, Viren oder Virulenzfaktoren zu messen.

Die Genauigkeit und Zuverlässigkeit des GI-MAP ermöglichen personalisierte Protokolle zur Behandlung von Darmfunktionsstörungen, basierend darauf, welche Infektionen dringend behandelt werden müssen, welche Bereiche des Darms bereits optimiert sind und welche Bereiche nach der Behebung einer Infektion behandelt werden sollten. Darüber hinaus bietet die Quantifizierung der identifizierten Mikroorganismen eine bemerkenswerte Möglichkeit, zu sehen, wie unter anderem, ob Behandlungsmodalitäten wirken, da ein Nachtest nach der Behandlung zeigen kann, ob ein Parasit beseitigt wurde, sich die Dysbiose verbessert hat.

Ablauf:

Ungefähre Bearbeitungszeit: 3 bis 4 Wochen

Sie bestellen den Test und bekommen das Test-Kit inkl. Vorbereitungs- und Durchführungsanleitung nach Hause geschickt. Sie vereinbaren mit DHL den Rückversand. Sie entnehmen die Proben anhand der zugeschickten Anleitungen. Sie schicken die Proben mit dem DHL-Beutel zurück ans Labor. Ihre Proben werden untersucht und analysiert, woraufhin ich einen Rapport erhalte, welchen wir gemeinsam, wenn Sie dies wünschen, in einer Besprechung (ca. 1h) anschauen. Die Empfehlungen besprechen Sie dann mit Ihrem Arzt Ihres Vertrauens, damit alles hinsichtlich bereits bestehender Medikation geprüft wird. Jegliche Haftung meinerseits ist ausgeschlossen. Wenn Sie Klient/in bei mir sind, kann die Besprechung in die Therapie eingebunden werden. Ansonsten wird der Aufwand und die Besprechung mit CHF 250.- verrechnet.

The Gastrointestinal Microbial Assay Plus (GI-MAP) was designed to assess a patient’s microbiome from a single stool sample, with particular attention to microbes that may be disturbing normal microbial balance and may contribute to perturbations in the gastrointestinal (GI) flora or illness. The panel is a comprehensive collection of microbial targets as well as immune and digestive markers. It screens for pathogenic bacteria, commensal bacteria, opportunistic pathogens, fungi, viruses, and parasites. It primarily uses multiplex, automated, DNA analysis to give integrative and functional medicine practitioners a better view into the gastrointestinal microbiome.

The Gastrointestinal Microbial Assay Plus (GI-MAP) is an innovative clinical tool that measures gastrointestinal microbiota DNA from a single stool sample with state of the art, quantitative polymerase chain reaction (qPCR) technology.

The GI-MAP was designed to detect microbes that may be disturbing normal microbial balance or contributing to illness as well as indicators of digestion, absorption, inflammation, and immune function. With a culture based test it is not possible to measure strict anaerobes, viruses or virulence factors.

The GI-MAP's accuracy and reliability allows personalized protocols to address gut dysfunction based on which infections are urgent, which areas of the gut are already optimized, and which areas should be addressed after an infection is resolved. Additionally, the quantification of identified micro-organisms offers a remarkable ability to see how treatment modalities are working because a retest after treatment can show whether a parasite has resolved, dysbiosis has improved, and more.

Process:

Approximate processing time: 3 to 4 weeks

You order the test and receive the test kit, including preparation and execution instructions, delivered to your home. You schedule the return shipment with DHL. You collect the samples according to the provided instructions. You send the samples back to the laboratory using the DHL return bag. Your samples will be examined and analyzed, after which I will receive a report. If you wish, we can review this report together in a consultation (approx. 1 hour).

You then discuss the recommendations with your trusted doctor to ensure everything is checked in relation to any existing medication. Any liability on my part is excluded.

If you are already my client, the consultation can be integrated into your therapy. Otherwise, the time and consultation will be charged at CHF 250.

Setzen Sie verschreibungspflichtige Medikamente niemals ohne vorherige Rücksprache mit Ihrem Arzt ab.

Altersanforderungen

Der Test kann in jedem Alter durchgeführt werden. Allerdings erscheint das Mikrobiom von Säuglingen oft sehr „dysbiotisch“, da sowohl das Mikrobiom als auch das Immunsystem des Säuglings noch unreif sind.

Medikamente

Wir empfehlen, alle verschreibungspflichtigen Medikamente vor dem Test weiterhin einzunehmen. Bitte dokumentieren Sie sorgfältig alle relevanten aktuellen oder kürzlich eingenommenen Medikamente. Medikamente, die die Zusammensetzung des Mikrobioms verändern können, sind unter anderem:

Antibiotika – Nach Abschluss einer Antibiotikatherapie sollte 4–6 Wochen gewartet werden, bevor die Probe entnommen wird.

Immunsuppressiva / orale Steroide – Diese Medikamente können zu einem niedrigeren Wert von Sekretorischem IgA, Anti-Gliadin-Antikörpern und Calprotectin führen. Nach Absetzen dieser Medikamente sollte 4–6 Wochen gewartet werden, bevor die Probe entnommen wird.

Nahrungsergänzungsmittel

Es ist nicht erforderlich, die meisten Nahrungsergänzungsmittel vor dem Test abzusetzen. Ob dies sinnvoll ist, liegt im Ermessen des behandelnden Arztes. Viele Ärzte/Therapeuten möchten die Wirkung von Nahrungsergänzungsmitteln auf das Darmmikrobiom verstehen und empfehlen daher, diese während des Tests weiter einzunehmen. Wenn ein Arzt eine „Baseline“-Analyse ohne den Einfluss von Nahrungsergänzungsmitteln bevorzugt, kann er empfehlen, diese vor dem Test abzusetzen. Einige Kategorien von Nahrungsergänzungsmitteln sollten besonders berücksichtigt werden:

Nicht verschreibungspflichtige antimikrobielle Mittel:
Obwohl es nicht erforderlich ist, Antibiotika oder antimikrobielle Substanzen vor der Probenentnahme abzusetzen, können diese die Testergebnisse beeinflussen. Falls möglich, empfehlen wir, 4–6 Wochen vor dem Test auf antimikrobielle Mittel zu verzichten.

Enzyme:
Die Einnahme von oralen Enzymen verändert den Elastase-1-Wert nicht. Enthält das Enzym jedoch Lipase oder Ochsengalle, kann der Steatocrit-Wert verringert werden.

Ochsengalle / Lipase:
Die Einnahme von Ochsengalle oder Lipase beeinflusst den Steatocrit-Wert, aber nicht Elastase-1. Wer den Basiswert ohne Nahrungsergänzungsmittel messen möchte, sollte mindestens 3 Tage vor dem Test auf diese verzichten. Wer prüfen möchte, wie gut die Therapie mit diesen Mitteln wirkt, sollte sie weiterhin einnehmen.

Probiotika:
Probiotische Nahrungsergänzungsmittel können die normale und opportunistische Bakterienflora beeinflussen. Wer die Basiszusammensetzung des Mikrobioms ohne Probiotika testen möchte, sollte diese ca. 2 Wochen vor dem Test absetzen. Wer testen möchte, wie sich ein bestimmtes Probiotikum auf das Mikrobiom auswirkt, sollte es weiterhin einnehmen.

Biofilm-Disruptoren:
Es gibt keine wissenschaftlichen Belege dafür, dass Biofilm-Disruptoren die Erkennung von Mikroorganismen verbessern. Einige Therapeuten setzen sie jedoch bis zu 14 Tage vor dem Test ein. Dies ist eine Option, aber keine Voraussetzung für den Test.

Preparation

Please refrain from taking aspirin for two days prior to collecting your sample.

Never discontinue prescription medication without first consulting your physician.

Age requirements

Can be used on all ages. However, often the microbiome of infants will appear very “dysbiotic”, as the microbiome and the infant’s immune system is very immature.

Medications

We recommend continuing all prescription medications prior to testing. Please carefully document any relevant recent or current medication use. Medications that may alter the composition of the microbiome include:

Antibiotics -(after finishing Antibiotics, wait 4-6 weeks before collecting sample)

Immune Suppressants/ Oral steroids- will likely cause a lower Secretory IgA, Anti-gliadin, and calprotectin result (after finishing these meds, wait 4-6 weeks before collecting sample)

Supplements:
It is not necessary to discontinue most supplements before testing. It is the provider’s discretion to recommend discontinuing supplements. Many providers will want to understand the impact of supplements on the gut ecosystem and will want their patients to continue supplements during testing. If a provider wants more of a “baseline” assessment using the GI-MAP, they may recommend discontinuing supplements prior to testing. There are a few categories of supplements that deserve consideration:

Non-prescription anti-microbial agents:
While we don’t require that people discontinue antibiotics or antimicrobial substances before collection their stool sample, doing the test while taking these substances can influence the test results. If possible, we recommend discontinuing anti-microbial agents 4-6 weeks before testing.

Enzymes:
Oral enzyme use will not change the elastase-1 finding. If the enzyme contains lipase or ox bile, it may decrease the value for steatocrit.

Ox Bile/Lipase:
Ox bile/lipase intake will affect the steatocrit level but will not affect Elastase-1. If you would like to see your patient’s baseline without supplementation, discontinue at least 3 days before testing. If you would like to see how well-managed your patient is on their supplement, do not discontinue.

Probiotics:
Probiotic supplements can influence the normal and opportunistic bacteria flora. If you would like to assess your patient’s baseline microbial balance without supplementation, discontinue for ~2 weeks prior to testing. If you would like to see the influence of the current probiotic supplement, do not discontinue prior to testing.

Biofilm disruptors:
There is no published evidence to suggest that a biofilm disruptor will improve detection of organisms. However, some clinicians choose to use a biofilm disruptor with their patients up to 14 days before testing. This is an option but not a requirement for testing.
